Help needed. Can anyone tell me what it says as place of death and where the informany lives on the image?

Many thanks for your help, much appreciated.
I think I may have found out through a bit of dectective work the name of the place of death and the place where the informant lived.
I believe the place of death is Ardeigh, Kildare (Ardee as of 2024) and the informant lived at Farm Hill, Kildare.
